If you wish to become to a new patient to our practice,  call 859-498-0200 or print off the new patient application form.  Please provide a copy of your insurance card with application. Please mail or drop off to the front office.

 

Once you are accepted, our office will call you with an appointment.  We ask that you print out and complete the NEW PATIENT PACKET and bring with you to your first visit.

• Picture ID

• All Medications (bring to every visit)

• Medical records from your previous physician
